Youth Survey

Students in grade 6,7,8,10,12 have been randomly selected to participate in an anonymous and confidential survey administered by AWAKE To a Safe and Healthy Community Coalition serving the Anthony Wayne area. The survey which will be taken during school hours, will be administered by AWAKE with the help of school guidance counselors.  The results of this survey will not only meet the needs of the grant for which AWAKE is funded, it will also help to identify key health problems and needs of our communities’ youth. 

This survey will meet the requirements of the Drug Free Communities Support Program and consist of questions from the Centers for Disease Control Behavior Risk Factor Surveillance System intended for students in grades 6 through 12.  You may click on the link below to see a blank copy of the survey or you may call Amy Barrett, Executive Director for AWAKE at 419-877-0466 ext. 12037 to make aggrangements to see a copy of the survey.
Students will answer health-related questions about smoking, drinking, drug use,personal safety and mental health. The survey and any reports about the survey will not contain your child’s name.  Students do not have to answer all of the questions and can skip any question they feel uncomfortable answering.  Students’ names or any personal identification will not be attached to the survey. 
​Although students will not receive any direct benefit from the survey, his/her participation may lead to a better understanding of the needs of our community.  Students are not required to take part in the survey, as their participation is completely voluntary. Students will not be penalized or treated differently if he/she chooses not to take part in this survey or later decides to stop participating.  Students will not be paid or compensated for completing the survey.
